Initial Comments:
Based on the findings of an onsite unannounced followup survey completed on December 20, 2019, Bradley Physical Therapy Clinic, Inc. had corrected the Condition level and standard level deficiencies cited under 42 CFR, Part 494.62, Subpart B, Conditions for Coverage of Suppliers of End-Stage Renal Disease (ESRD) Services - Emergency Preparedness. The deficiencies were cited as a result of the findings of a Medicare recertification survey completed November 21, 2019.

The deficiency was cited as a result of a Medicare recertification survey completed November 21, 2019.
